Duct Tape allows you to tape up a puppet so it no longer respawns when you leave and return to an area.
Simply finish the game completely without using any Duct Tape to gain this achievement (you can't leave early, or use Free-cam Mode and go to the final room in the hotel and leave).
I'm not sure if modes Cancel out achievements or not, but on my 3rd playthrough I used the following cheats:
Infinite Ammo
Mega Wrench
Speedy Gordon
Fast Weapons
Invincibility
And was able to mop up the achievements I had missed previously, and other people have reported they also got achievements with these cheats on, So you can get this with these cheats enabled.
