Pretty straight forward take the final Blue Page from the fireplace and insert it into the Blue Book. As soon as you insert the page a loading screen will appear and the achievement will pop.

If you're having trouble with this one, recount the pages on your shelf. It wasn't popping for me so I thought I forgot to get the fireplace page. I kept reloading the fireplace 158 code and no page appeared. So I looked at my shelf and realized I only had five blue pages and hopefully I missed one in one of the other worlds (thinking maybe I grabbed a blue one first and then a red one and it maybe only let me carry one page and dropped the blue one automatically?) not sure what happened but I re-visited all of the worlds and finally found the sixth blue one in the Woods world to the left of the third floor elevator. Good luck!
