After killing your first few groups of enemies you will come across a floating bundle of grenades at the end of a platform. Once you pick them up you should have 6 at your disposal. To use a grenade hit 
and you will toss it about half a screen in front of you.
(The grenade is thrown whichever way you are facing, so make sure you are looking at a group before hitting.)
The grenades have a nice blast radius, so when a group of enemies appears just throw a grenade their way and they should all be done for. You will come across random floating grenades during the rest of the level so don't worry about running out, just get your achievement.

. Getting 20 kills should occur during the first mission, as you will in fact get hundreds of grenade kills throughout a single playthrough (unless you don't use them, for some reason).
