You will receive the Big Narita upgrade in the 'Chapel of the Techno-Fathers'. To access the Chapel, you'll need to fight your way through the Thermal Baths to receive the key from the NPC on the balcony. Exit the balcony, then exit the Thermal Baths and head right. Use the key in the 'Yellow Quarter' terminal to gain access to 'The Bar of the Thirsty Horseman'. Speak with the robed NPC at the back to receive the key to the Chapel. Exit the bar, use the key on the terminal to your right to enter the Chapel.
Assuming you visited the balcony previously in the Blue House, you'll receive Deja-Vu for returning a second time.

Exit the chapel, walk left to the robot in the middle of the street and interact with it. You will begin to transform into Big Narita and the achievement will unlock as the tutorial shows up.
