How to unlock the Boruto's Tale - All S Ranks achievement in NARUTO SHIPPUDEN: Ultimate Ninja STORM 4 - Definitive Guide
Okay so i'm doing this to reiterate the solution of 'Mr Bones7' and the comment by 'BrutalGuitar' so that it's clear to everyone (plus adding in some of my own stuff).
You go to the Amaguriama shop close to the Hokage's place (at the beginning of the main strip - up the top) and purchase the 'Amaguriama Deluxe', keep purchasing until you get the added rare ability (it'll be in red) of Health Regeneration (Large). Each purchase will nullify the previous so they won't stack up per each purchase but the Health Regenerate (Large) is all you need!
Now press and click 'Main Event Info' to replay the past main event battles that you want to get the S rank in. The Amaguriama lasted 3 battles for me and worked with any character (I was initially worried that it would only work with Boruto but it works with any character for any battle regardless).
It's much easier to wait till you've finished the main story-line so that money isn't a problem and so you can go back to complete the ones you haven't done and do them all at once with the Amaguriama boost at your dispense and chances are you've already got an S rank in a few battles from the initial completion so that just makes this easier.
When you're in battle you will see how fast your health regenerates and begin to question how this is even fair , in case you're struggling just get the enemy down to as low health as possible and then let your health regenerate fully (to play it safe) and then hit the finishing blow. Keep using substitutes and chakra dashes to move away from the enemy to let your health replenish. Remembering that every second you remain unscathed you're recovering a lot of health so this shouldn't be a problem.
I also didn't finish a battle without full health and I didn't stand by waiting for my health to regenerate, I just beat them as quickly as possible and my health regenerated in the meantime.

I'm stuck at 90 percent even though I have s ranked everything oddly enough ugh this is annoying

Finally unlocked I had to replay the last mission I S ranked and S rank it again

Just a helpful hint all story fights can be easily S ranked by using the food buff. I just bought the 5,000 ryo one until I had health regen large.

I assume you're referring the Amaguriama shop where you buy the Amaguriama Deluxe for 5,000 Ryo. It gives large boosts to strength, defense, and a few other thing. It's located close to the Hokage's place. You might want to update your guide to make it more specific and clear. That boost really does help alot.
