Pickup whatever team you want with both controllers.
Set up 3pt sliders to 100 and steal success to 100. Pickup one player that you want to have five by five and simply score 5 points, get 5 steal, assist 5 times, rebound 5 times (shot half or fullcourt shots with other controller), and block second controller 5 times ( simply press X with second controller and instantly Y with your first controller).

The only setting that matters for this achievement is the quarter length. 5 minute quarters are minimum for this achievement.
Other then that, you can turn off all NBA rules, and turn User/Computer Sliders up/down.
5 points, 5 rebounds, 5 assists, 5 steals, 5 blocks.
The only problem you should have with this achievement would be the blocks.
Turn Shooting fouls off. Put User Contest shots slider all the way up. Turn CPU take inside shots and close shots sliders all the way up.
Control the point guard, walk away from the man you should be guarding, baiting him into driving the lane, once he begins his drive switch to the PF or Center. It may take a few tries, but the blocks will come.
*Another method here is to use two controllers. Player 2 use the 92-93 Hornets(unlocked in greatest mode) Player 1 can pick any team. 2nd player shoots but holds the shot as long as he can with Muggsy Bouges. Player 1 wants to be there in front of Muggsy for the easy block, just jump to block the shot.*
