How to unlock the Perfect Practice Makes Perfect achievement in NBA 2K15 - Definitive Guide
This achievement is either gonna be either entirely frustrating or entirely easy. A few things factor into the difficulty to this achievement. First of all, you can only get this through quick games and maybe myCareer games. You CANNOT get this in myTeam. I have had about 8-9 perfect releases in a myTeam game before and no achievement. Next, you need to figure out the latency to your television. I actually used a computer monitor with relatively no lag. Once you figure out around how much lag your output source produces, you then need to find one or two players (preferably on the same team) that you can pretty much figure out their shots. For me, I used Golden State. I pretty much know Andre Iguodala's shot and Curry/Thompson's shots are pretty simple to gain the grasp on. But if you have one guy or two you know pretty well use them.
Another thing that may help is you can edit players. So when you find a shooting stroke you are good and familiar with, change every player on one teams shot to that shooting stroke. You can then move the ball and get this with ease.

I can confirm that this can be unlocked in mycareer and rookie difficulty.

I did it more than 10 times in a game but the achievement is stuck at 24% :(

This trophy can only be earned in quick games. I recommend putting it on Rookie difficulty because it makes your shot easier to time. Use a player or multiple players that you are good at shooting with. If you aren't good at timing anyone's shot, a couple players who have an easy to time shot are Mavericks SG Monta Ellis or Wizards PG John Wall.
