Simply set the 3 point slider to 100 and set the difficulty to rookie and get into a game. A fast-break is when you rebound the ball and head over to score. Just grab the board, run over to the three point line and pull up that shot with 100% chance of going in and once you hit it, it should pop the achievement.

When you are on a fast-break (opponent turns the ball over and you are in transition to your basket) simply make a 3 pointer. Since it is a fast-break basket you might be off balance when shooting so I recommend putting the 3 point slider to 100 and put the game difficulty to Rookie.
