Just select the option to create a player on the main page. As soon as you are done customizing (you don't have to change a thing but can if you wish) save the created player and the achievement will pop up.

When you are in the opening practice menu press 
and go to the menu. From here go to create a player. Then customize your own player and when you finish you will get an achievement.
