Super easy way to unlock this is....
a) Start a dynasty with a team in a weaker conference.
b) Edit 5 players to have 99 stats in everything and make sure you put them to start.
c) Simulate all HOME games with ESPN Gamecast so you make sure you win, you can either enter the game by clicking Y, and click B to exit the Gamecast.
d) Do this until you win all your home games and once you reach the off-season, this will unlock!

Also while going for Hardcore, you need to win every home game during a single season. Home games are identified by a blue color in the upper right corner of the game on the calender. You will be on the right on the team select screen if you are the home team as well. You should turn Autosave off in case you need to replay a game. Again, if playing every game, make sure you win. If you are simulating, save before every home game and repeat the game if you lose. The achievement unlocks once you start the off-season after winning every home game.
