First, make sure that you are playing a game with 5 minute quarters on Varsity difficulty. This achievement will not unlock if you are playing shorter quarters/easier difficulty (personal experience). It is also a good idea to use a team like Ohio State (which is rated A-) versus San Jose State (rated D+).
A shutout is accomplished when you prevent the other team from scoring any points.
The biggest favor you can do for yourself prior to the game is to adjust the offsides slider so that offsides is turned off. During the game, this will allow you to place a defender right next to the quarterback and sack/tackle him immediately after the ball is snapped.
BIG TIP - If the quarterback lines up in shotgun, stand between the center and the quarterback. When the ball is snapped, it will result in a fumble that you may have an opportunity to recover, thus resulting in the CPU having the ball less time.
When you have the ball on offense, stick to mostly rushing plays in order to run down the clock. Also, it is a good idea to dive every five or so yards in order to keep the clock running and to give yourself more opportunities to run down the clock. Do not be concerned with running up the score, focus on running out the clock and keeping the CPU from scoring.

I've done this multiple times now playing on the hardest difficulty with 5 minute quarters and it still won't unlock...

Done it 8 times straight on heisman 5 minutes quarters. No achievement

Set up a shortened-quarter game in Play Now mode. When you have possession of the ball, let the play clock run as low as possible on each down before snapping the ball. Continue to eat up as much clock as you can on offense.
On defense, turn off the penalties slider and position a defender next to the opposing quarterback, being careful not to make contact with any other defenders before the ball is snapped. You should easily force a three-and-out so that the CPU doesn't get a chance to score.
The Achievement will unlock at the end of the game.
