This one is kind of random, since you can't do anything directly to win the achievement; the "writers" vote for the Heisman winner at the end of each season, so even if one of your offensive players has a great season, a player on a CPU team can still win the Heisman.

What I did was create a maxxed-out quarterback prospect in Dynasty Mode, and recruited him to my team (USC). If you're going to try and win this achievement with a quarterback, try and have as many four and five-star wide receivers on your team as possible; so recruit as many as you can. If you have to, create a bunch of maxxed-out wideouts and then recruit them; chances are you'll end up with a couple.

While you're busy creating superstar wide receivers and quarterbacks, make and recruit a couple of maxxed-out offensive linemen too, preferably tackles. They'll give your QB a lot of time to throw the ball.

With a five-star QB, a couple of five-star wideouts, and some monster linemen on a high-profile team, I then simmed my way through a season with USC and won nearly every game in a rout. At the end of my quarterback's sophomore season, he won the Heisman and I won the achievement.

Once again, this one is random and what I did may not work for you the first time. Even doing it in the way described above, it still took me a few seasons before the achievement unlocked.

I noticed after starting several different dynasty's that it seemed Hawaii always had the Heisman winner the first season. Just start a new dynasty selecting Hawaii as your team and sim the entire season. I got it the first try but if it doesn't just try another new dynasty and sim again. Be sure to advance until the winner of the trophy is shown because even when you don't win it the winner is shown.
