If you have possession of the ball on your opponent's 1-yard line, you can easily obtain this Achievement.
Though it sounds like you need to run a fake punt play to unlock this, you can pause the game and edit your depth chart to substitute your punter in at the QB position. Select a QB Sneak play, and you can pretty much just fall forward into the end zone for the touchdown and the Achievement.

People also keep asking me how I got this achievement, it is actually quite easy. Drive down the field to the 1 yd. line. Once you're on the 1 yd. line, go to:
-Pause Menu
-Strategy
-Depth Chart
Then put you P at the QB position. After that, go the Goal Line plays, select QB Sneak, after you select that play run the ball in the endzone with your Punter. After you score with your Punter the achievement will unlock.
