A Red Zone Efficiency rating of 100% means that every time you have possession of the ball inside the opposing 20 yard line, you score a field goal or a touchdown. (You have to execute at least one play from scrimmage inside the 20 - scoring on a play from 21+ yards out doesn't count as being in the red zone.)
If you're having trouble unlocking this one, you can do all sorts of things from adjusting sliders, to lowering the difficulty level, but the key is once you score on your first trip to the end zone, you have a 100% rating. Just stay out of the Red Zone for the rest of the game and you'll unlock it. Each time you get the ball, you can run out some time off the clock and punt the ball away. Just be careful not to force any turnovers in your Red Zone and you're golden.

Every time you get into the redzone (inside the 20 yard line), you have either get a TD or kick a field goal. If you turn the ball over, miss field goal, or the clock runs out on you, that counts as a redzone attempt without points. This is made easier if you play as a powerhouse school against a creampuff school.
