For this one you need to start a Dynasty with any team. If you don't want to play the whole season you can sim the entire season, past the bowl games to the Off season. From there you need to sim past everything in the off season, all the way to the "custom conferences" tab. When you are there click on custom conferences and move a team from one conference to another or just make a minor adjustment and you have the achievement.

During the off-season (after the completion of a season), during Week 13, there's an option to customize conferences. During that week, go to Custom Conferences and simply swap one team from the conference you're in for another team from another conference

After finishing a season in dynasty toward the end of the off-season, you will be given the option to edit conferences. Simply add a team to a conference to unlock the achievement.
