In a new Dynasty, go to custom conferences in the setup screen. Add teams to any conference until there are 16 (easiest to start with Pac-12, ACC or Big Ten which start with 12 teams). Once the conference has 16 teams, the achievement should pop, and you can then change it back or cancel or whatever you like.

I think I'll take the Big Ten and add Maryland, Rutgers, and maybe Notre Dame and Ball State.

See step 3 in the Road Map for the fastest way to obtain this trophy.
When creating a dynasty, go to Custom Conferences. Press 
and choose the ACC. Choose Add Team and choose any team to add to the conference. Do this three more times and there will be 16 teams and then the trophy will pop.

After creating a new dynasty, you will be able to edit your dynasty. Simply select Custom Conferences option, select a conference, and add 16 teams.
