Under Team Management, Go into create a playbook, pick any playbook, hit b to exit it will prompt you to save and exit or not save and exit, chose save and exit, save it, acheivement pops

Custom playbook is under "Team Management" in the main menu.

From the Main Menu go to Team Management > Custom Playbook > Choose either Offense or Defense > Select a Team > Scroll down to Rename Playbook > Save by pressing

From the Main Menu, go to the Team Management tab and select Custom Playbooks. Select if you want to do offense or defense then select a team. Add a play or form and the achievement will unlock.
