First you require a maiming injury to replace with bionics.
The game is comparatively easy owing to the poor AI so in regular play you may struggle to get any injuries at all, let along a maiming injury (losing a limb, eye etc.) which are a comparatively rare subset of injuries (around 5% it seems).
The easiest way to do this is to simply hire 5 green Juves and enter a match (skirmish with AI works with a custom match, or an operation on deadly) and simply forfeit as soon as you can to acquire injuries. (all gangers roll and injury for a forfeit). This should come relatively quickly. Note though that mine did not unlock with the first maiming injury I received so it may be slightly buggy.
I;d suggest you do this with a fresh gang. Make sure it isn't your first ever gang outside story mode and instead wait until you build up some infamy with your main gang (this comes naturally when playing for the other achievements and is available across all gangs). When you create a new gang hire the max 3 Juves and and leader and select an operation on deadly with bonuses to level 5 weapons and armor (takes 60 infamy, 30 each for max level) with any left over in lower level items.
After this go to the shop, open the crates and sell them and all your starting equipment. This will leave you enough funds to hire a 5th Juve and to afford a bionic implant.
Per the above play until you have a maiming injury and then go to crew management, select that ganger and LT until you get to the injuries tab. You can then purchase a bionic enhancement on the severed body part to unlock this achievement.
