The game is comparatively easy owing to the poor AI so in regular play you may struggle to get any injuries at all, let along a maiming injury (losing a limb, eye etc.) which are a comparatively rare subset of injuries (around 5% it seems).
The easiest way to do this is to simply hire 5 green Juves and enter a match (skirmish with AI works with a custom match, or an operation on deadly) and simply forfeit as soon as you can to acquire injuries. (all gangers roll an injury for a forfeit). This should come relatively quickly.
Note though that this did not unlock for me twice when losing a right leg, but did when a ganger lost an eye, so it may be glitchy.
