This can't be done on EASY mode.
Start the game as Wade (default character), as he starts with a grenade automatically. Enter the abyss, find a room with enemies, and let them get you down to half a heart. Then run to a wall, stand right beside it, and press
to throw your grenade into it. The blast should kill you, and the achievement will pop as the screen turns black.

Adding some additional info, it seems this solution does not work on easy difficulty, as your own grenades do not damage you. Switch to normal and the above solution will work just fine.
