How to unlock the Sherlock achievement in Neon Abyss - Definitive Guide
5 in a row means 5 levels in a row when you found the secret room. I found all the secret rooms in levels 1 to 5 but 2-6 may work. You'll need grenades or explosive ammo to blast your way in to a secret room. On the first level, you'll often see part of a wall glowing yellow; that's your way in. Hit it with a grenade or explosive ammo and you can enter. After that, you'll need to work out where the entrances are.
The entrance is always in a teleporter room and there has to be 'room' for the secret room to appear on your map. So, if you can see a teleporter room has 4 rooms coming off it already, 2 at the top and 2 at the bottom, there cannot be a secret room. Also, there must be easy access. There will never be an entrance half way up a wall for instance, so you can usually work out which walls to throw your grenades at. There are no telltale signs I'm afraid, so if there is more than 1 option, it's pot luck.
If you start with Wade, he'll always begin with 1 grenade but after that, it's up to you to find or buy at least 4 more. However, there are several guns which have explosive ammo or can be modified to add rockets etc. This removes the need for grenades and gives you unlimited tries. I found the best gun was the Frostfire as it charges whilst you are firing and when you release, it shoots an ice grenade. The best thing about it is those grenades don't harm you, unlike rockets etc if you're too close to the explosion.
The first time I did this, it did not pop but the second time it did. Might be glitchy or I may have miscounted. Good luck!

Something important to note is that if you run into a "?" level, then you will not be able to get this in that run. I usually get it after level 4 before going into 5 and as far as I can tell you cannot fund any secret in that level, I used grenades in every teleporter room and couldn't find any. Also this could be wrong but its possible that quitting to the main menu and resuming might screw this up, I swear this took multiple tries to unlock (Windows version).

There is a item, I forget what it was called but it opens all secret rooms. I got that on the first level and it was just finding the already blown open wall in each level.

This achievement requires you to find and enter the secret room on five floors in a row. Every floor has a single secret room, opened by blasting the wall with an explosive (grenade). There are skills in the skill tree to reveal the wall in question on the first and second floors of each run, but you'll have to find the secret rooms yourself on floors three, four, and five.
This is way easier to do than it sounds. Most important is making sure you actually have the grenades on each floor to blast open the walls. Playing as Wade starts you with a grenade to guarantee you get the first floor secret room, and you should have no problem getting at least one grenade per floor for the others.
As far as finding the secret rooms, they're always in teleporter rooms, never in a random single room. teleporter rooms can have up to four exits from them, and the secret room will always be in one of those four standard positions (upper left, upper right, lower left, or lower right). Explore each floor and collect all the consumables, then circle back to each teleporter room and throw a grenade at each wall in each teleporter room where there isn't an exit marked on your map, to see if the wall blows open. If you're short on grenades and don't have many to waste looking around, you can find where the secret room is after using multiple grenades, then use the Save Reload Exploit to start the floor over. The secret room will always be in the same spot, allowing you to open it with a single grenade and save the others for the next floor.
