How to unlock the I'm Fine achievement in Neon Abyss - Definitive Guide

When you earn enough gems (you get one for killing a boss at the end of each level) you can spend them at the bar on a skill tree. Once you've opened enough you'll get more trees, On the third tree, you can unlock a character called R-6, for 30 gems. This character only has shields, no hearts, so if you can complete a run with him then the achievement will pop. Doesn't matter it you try to pick up any hearts or get items that should give you hearts, you won't get any.

It's also easier to get the 10 shields achievement with this character too, just because you start with 3: -

I can confirm that the pet "Blue Doctor" whom eats hearts and drops shields does NOT void this achievement.

In the fourth tree there is an item pack called Howard reactor , it will replace all hearts by shields , use it and complete a run , the achievement will pop .

I had a run that gave me a few items that gave me shields for doing certain actions, Like picking up coins or activating fast travel runes.
I put the game on easy, Then started the run avoiding hearts. After the first floor is when I started getting the shield items and by floor 3, I had 10 shields.
Every boss can be beat without taking damage, and if you watch a vid on youtube before fighting them you'll give yourself a better chance of making it to the end.
SinFluxx has added that picking up items that add heart containers doesn't void the achievement!

Do you know if picking up an item that gives you an extra heart container voids this?

Answered my own question, picking up items that give extra heart containers is fine to do.

to avoid accidentally picking up hearts you can do it with the robot character as he has zero hearts anyway. i think i even picked up one or two molded hearts, but i still got the cheevo in the end of the run

"Complete a run" means what exactly?

Finish the run up to your current target, e.g. if you haven't killed a target yet then you need to complete up to and including stage 5 (killing Argus).

So if you've already progressed past Argus on your main save file it's worth picking a different save slot so you only have to do 5 stages rather than 7/8, depending on how far you are.

When you start the game change your save slot instead of choosing "Enter the bar" first and you can then attempt this on the first target on that save slot, and just go back to your main save slot after.

I'm sure you can do it if you've already progressed, but seeing as every extra stage is another where you take a full heart of damage it definitely makes it harder than it needs to be!

Refer to Safety First (80G) for more information. You'll unlock this naturally while getting that achievement.

I advise you to choose the cyborg character R6, he initially only has armor and does not have the ability to collect hearts. By the way, you can take items that add a permanent heart.
