As you go through the stages and pick which of the 1 to 3 battles you want to start at each stage there are missions attached to each. Some missions are fairly easy like play 10 cards during a battle or buying 3 items at the store. Some missions can be quite difficult like clearing 2 elite battles without losing life unless you consciously go for it.
For this achievement you must clear all of these by the end of the game whether you are using the Unknown Stone or not.
Some helpful reminders are as follow.
-You do not need to clear the mission on the battle you take it on. You can clear it anytime before the end of the game and it'll count. i.e. Buying 3 items at the store is probably not possible in the first battle or two until you get enough cash.
-There are not any missions in round 3-5 and beyond.
-Try picking the easier missions rather than the missions that give the best rewards if you are having trouble completing this.
As a final note you will probably pick this up sometime during your run to 1000 rounds as you get more familiar with the game and can complete it (without the Unknown Stone) in around 20 minutes getting from stage 1-1 to 3-5.
