You can take damage. You have to have max HP after each fight to get this achievement.
7 card deck i used for it:
>Powerful Strike (upgraded included in base deck)
>Vampiric Strike (upgrade not needed as you will have 4 cars anyway)
>Aura of Darkness
>Time of Blood (upgraded)
>Blood lust (included in base vampire deck)
>Reserved Force (upgraded) (optional, beat the game wihtout it as well)
>Vampire (upgraded)
skills:
>Aura of Blood (+2 mana when you have vampiric statate)
>Save Mana (you no longer lose leftover mana when ending turn)
items: ai white candy (to remove and upgrade cards)
if you have those 7 cards then you start first turn with Vampire -> Aura of Darkness -> Reserved Force
next turn and every single turn after that you only have 4 card in your deck so i usually go for Time of Blood -> Blood Lust and then if i just mindlesly aoe i go for Vampiric Strike or if i want to focus one dude first i go for Powerful Strike
Skills that give you gold like Devils Advocate and Windfall are nice. Bloody Explosion is additional 3 dmg per turn so its also ok if you have it in your tree. Endless Challenge can help you use your banked mana
You will just face tank everything and heal with Vampirism.
As long as you have full HP after each fight you will get the achievement. You dont have to play on Trascendence mode. Do it in normal Adventure mode

I did this with Helena on my first try. Fill your deck with HP cards, and rely on your guardians to do damage.
Also buy HP releated items like regeneration and heal. Hold on to them for emergencies.

Hi! In case of anybody is wondering: you might not run in those cards, since many of those need to be bought or given to you during the run. So look out for similar cards. The deck build works though!
