How to unlock the Ancient achievement in Neverwinter - Definitive Guide

Once you're in your 20s, you can get a quest from Sergeant Knox to learn about artifacts. I got it when I was 24 but it's marked as 21 so you may be able to get it then.
For this quest, you go to the artifact guy in town and then he has you do a solo dungeon. It's a pretty straight-forward place. You get through the initial area, and then you activate a skull to create a bridge for you. Then it just repeats; fight, bridge, fight, bridge, etc.

The last platform has a guy who asks you a question. If you answer it correctly you don't have to fight him. According to the wiki, the correct answer is "The Armies of Neverwinter."

At the end you loot a box. Bring it back to the artifact guy and choose which one you want. Equip it and bang, achievement.

This achievement may be glitchy at first, I got it last night and no pop, so today I went in unequipped it and the reequipped it and the achievement popped.

I will point out this was very difficult for me to finish alone. It could be my weapons, but I had to respawn twice.

Yeah this did seem to have above average difficulty compared to many of the other quests in the game.

What the red skull mean's?

Red skull as far as I can tell means it's an enemy that scales based off your level. Which means it shouldn't matter what level you are it will be the same difficulty.

This trophy unlocks when you equip a Artifact from your inventory. You can either buy a Artifact from the Trade House/Zen Marketplace, or complete the special quests at lvl 21 "Artifact Facts" and "Artifact Recovery" to get a free one.

Upon reaching level 21, head to Sergeant Knox who will give you the "Artifacts Facts" quest which leads you to the Sage's Shop. There you will find Elder Reader Laris who will give you the “Artifact Recovery” quest. Finish the quest, acquire the artifact and once you equip the artifact of your choosing, the achievement will unlock. Artifacts are very rare in Neverwinter; the above quest is your best bet to knock out the achievement with ease! Most other artifacts either drop from Dragons, end game zones, or lockboxes.

An Artifact is a piece of equipment that gives you stat bonuses for a short time and can be used generally every few minutes. They range from common to legendary and can earn ranks via refining enchantments and runestones into them. As they earn ranks via enchantment the refresh time is reduced and the stat bonuses increase. You can equip up to 3 artifacts, but you can only use one of the effects for the primary one you have equipped.

At level 21 , the required quest is given by Sergeant Knox.
The quest is called “Search for Artifacts” and further along the chain “ Return of the Artifact ”.
