Before trying to get this achievement, first, you must achieve at least level 49.
Soon after completing the area "The Chasm" will get access to the Dungeon "Spellplague Caverns".
Start and progress in the dungeon until you reach and defeat the first boss ("Plague Reaver"). In this same area there will be a hidden area on the left side where there is a Orb ("Orb of Blue Fire")... take it and then continue again until you reach the next boss ("Mouth Of Madness").
After defeating the boss "Mouth Of Madness", starting from the entry of the same area, go right and you will find another "Blue Orb of Fire"... take again.
A little further on you will find a cliff and down a camp on the right. At this point, if you look back you will see three stones with blue symbols. Place the Orbs in the rocks on the right and left side and the middle will reveal a path.
Follow the path and after defeating some groups of enemies will reach the area where is located the secret boss "Oculus the Inflamed".
To unlock the achievement you will have to defeat this boss.

Id anyone is wondering this is very much soloable post 60. Iv just done it with a gearscore of 9200, rogue.

I'm level 49, I didn't complete 'The Chasm' yet. I can access the dungeon, you will need to update achievement guide details.

Thanks for the solution! Also, its noticeable in the video, but the secret door is at the bottom of the last drop right before the end boss. So if for some reason you hit the boss bonfire and haven't found the path entrance, just turn around and head back to the bottom of the cliff. Its perfect if you have a party that is insistent on running past all the mobs and rushing to the last boss since you can then corral them at this point.

I was able to access this in the queue list of stuff at level 47 and queue to get in with randoms without having ever been to the chasm. It will scale you to 51 making you a burden on the others but if there are clerics it will be easy enough.

Took over an hour to fight to the the boss in the low level 50's and a few messages to the ramdoms to help me after I had picked up the orbs and opened the door but the boss it self wasn't too difficult.

Just tried on the epic version - the enemies were attacking from behind the force field thing and I was unable to enter.

With the new patch the instance is no longer there, this boss is now in front of where the entrance of the instance was ready to be killed. (it's easy solo)

Confirm DG's info. Called and reset my char after the patch and there it was as a random enc. I was a bit confused but I'll take it. Easy 15gs.

We hit the bug where we couldn't bypass the force field to enter the boss area, but he could attack us through it! We ended up having to kill some random mobs by the campfire so the game wouldn't see us as "in combat." After that, I even died once and was able to reenter.

You want to go to The Chasm , the jagged approach, 14333, -853 . Boss is now a random fight. Hope that helps.

also use Stefan's tips .

Purely for anyone else, since I had a terrible time finding the place again (had already done the quests in the area, and never encountered him). To get back to him use the portal at -13266,-348 to "Scrying Tower", then the one at -506,420 to "Unhallowed Grounds", and then the portal at 154,999 to "Mystic Grotto", then you're in the right area to go and chase him down.

thanks man! for everybody who doesn't know where to check your coordinates, the boss is to the right of the last dungeon entrance in the chasm when looking at the doors :)

since the dungeon isn't in the que anymore I had given up on getting this achievement. this solution really helped, and its a really easy fight for a lvl 70 character.

Beware outdated guides that don't tell you where to go for this Achievement! As of June 2016, this guide is accurate.

This Achievement is for defeating the secret boss Oculus the Inflamed. He can be found way at the end of The Chasm, a Level 49 area. So you must be Level 49 or higher to get the Achievement.

In The Chasm, take the portals at the following map coordinates to reach the Mystic Grotto/The Jagged Approach where Oculus appears:
-13266,-348 portal to Scrying Tower
-506,420 portal to Unhallowed Grounds
-154,999 portal to Mystic Grotto (head Northeast to find this portal)

Oculus can be found in a hidden area that appears as a darkened tunnel at the top-right corner of the map. This area is known as The Jagged Approach, but you reach it on foot from Mystic Grotto. If his location is not randomized, you'll find him at the Northeast corner of the map near 14137, -902.

Just beat the boss and the Achievement will pop.

Eye Chirurgeon

The other guides confused me so I made this guide to help others.

I found Oculus the Inflamed in THE CHASM final area called "The Jagged Approach" in the top right corner Coordinates 14319, -831. Kill him and the achievement will pop.

His new location is to the right of the Twisted Fane in the Chasm a few yards down. It might help to look for him when you get the Source of Corruption quest to make tracking Oculus easier.

i found him around a winding trail in the woods in jagged approach, i was killing the giant size enemies thinking he spawned in thier place, my exact location was 14311, -932

The guy you need to defeat for this trophy is Oculus the Inflamed. He used to be available through collecting three orbs then taking the orbs to the correct location to summon him - not anymore. Now this guy just spawns on the map.

He's in The Chasm/ Jagged Approach. At a very far end of the map where it v's into a dead end. The right path doesn't track on the map, but following it to its end will take you to the boss. The following video shows how to get there, but starts at the Unhallowed Ground part of the Chasm map.

In regards for tips, just wail on him. He does large AOE attacks, so back away or guard when the red circles appear on the ground - he unleashes a blue flame attack after a few seconds. Ranged characters may want to bring a companion that can bait him. If you are over level 60, you shouldn't have any problem fighting him.

This encounter has completely changed since the release of the Elemental Evil expansion. The Spellplague caverns dungeon no longer exists; however, Oculus the Inflamed has been migrated to the Chasm zone itself as a random encounter. The Chasm still has a recommended level of 48-52.

Oculus can now be found at the following coordinates: 14337 by -840. This is the third major section of the Chasm (after teleporting twice). You must then reach the 'Jagged Approach' area and travel to the upper left corner where you'll find a fork in the road; head right. At this point, you actually go off the map to the coordinates listed where you will find Oculus. If he is not there, just wait around as his respawn timer is pretty quick at four minutes.

As for the fight itself, Oculus is low 50's in level and shouldn't pose much challenge to a mid 50's player. I tested him at level 66 and he was dead in less than 10 seconds; he also did not aggro me when standing next to him. He did not drop anything special. It's much easier to get this achievement now than before. So if you put it off, look him up, he is dying to see you.
