As the achievement says, head to Dead Man's Point. Look for a grey partially destroyed stone tower. If you set Dead Man's Point as your waypoint, the tower is just south of it along the western zone wall. Inspect the skeleton in the rubble to pop this. Here is a shot of the tower's location on the map:

EDIT: Complete the Crazed Exile achievement 1st, he will banish you right to where you need to be for crushed skeleton.
The map coordinates for me was 123, 1879.

The spot you're looking for is in the 'Dead Man's Point' area of the Cold Run zone. This is the mid-western section of the zone. You will come across a crumbled, stone tower and in the midst of the ruins is a skeleton of a doomed traveler. The coordinates are: 120, 1883. Inspect the skeleton and earn your achievement.
Oddly enough, if you do the "Crazed Exile" achievement first, Caliban may transport you to the top of the crumbled tower. Refer to that achievement below for more information.
