Okay, so the achievement description is a little misleading. To obtain this achievement you must obtain the "Flayer Slayer" title on a single character which is obtained by killing the Illithilich boss in Throne of the Dwarven Gods 25 times in either game mode.
These 25 kills only count towards the title/achievement if done with a single character. You cannot get 20 kills on one character and 5 on another and expect the achievement to pop.
The achievement tracker does not show your progress towards this achievement but you can go into your in game Journal and go to the achievements tab and look under Slayer, show available and look for Flayer Slayer.

Illithilich is found in phase three, of either variant, in the 'Throne of the Dwarven Gods' skirmish. Successfully complete the skirmish 25 times; the medal you receive does not matter. You must defeat the boss 25 times on one character, not across multiple.
