How to unlock the Foulspawn achievement in Neverwinter - Definitive Guide
This took me ages to find the location of foulspawn enemies but the best place i found them was at the chasm and go through the teleport to apprentice row, i just went round that part of the map killing them every time they respawn. Hope this helps everyone :)

Just adding to this, I found two spots with quick re-spawns right as you enter Apprentice Row. Just as you enter it and to the left there is that glowing portal from the one quest you do here and then just past it up the hill is a second. I found them to have fast re-spawns. Watch out if they conjure up some aberrations, so that will help the the other one if you have not gotten it yet. Plus there is on patrolling soldier foulspawn on the upper tier to catch on occasion.

This can easily be done during the quest "Spellburst" when you are required to investigate Beggar's Nest.
A portal will spawn not long after you meet Josef. The portal will spawn both Foulspawn and Aberrations; therefore when using this method you will also get the Aberrations achievement.
All you have to do is stand killing the enemies without destroying the portal, so make sure you don't destroy both sides of the portal (one side is fine).
A healing familiar also helps as you shouldn't have to use a single health potion this way.
Hope this helps :)

The counter appears to include BOTH of your characters. I got this achievement while leveling up my second character.

The Plague Tower will also spawn a lot of Foulspawn. Might be worth noting this quest as well.

I want to add that if you have already solved those quests mentioned in the solutions above and you miss only few foulspawn kills but dont want to start over with a new character to get those quests again ,you can enter the skirmish battle Gate Crashers in the Chasm by speaking to Corporal Morgan next to the first bonfire.
Foulspawns will appear there aswell.

You'll get this trophy for killing 500 enemies classified as Foulspawn. You'll know they're Foulspawn because the top line in the enemies name will include the phrase, example: "Foulspawn - Berserker".
The best places to find Foulspawn are during the Spellplague Questline and in the Chasm adventure zone in the final area called the Jagged Approach, near the steps to the final Chasm dungeon Twisted Fane
To check your progress on this trophy first press the Options button > Quests > Journal > press R1 to go to Achievements > Slayer > hit to show them if they aren't visible and scroll down with R-stick to the Foulspawn Slayer entries.

Foulspawn are ‘deranged’ humanoid type enemies that originate from the Far Realm. There are many different types including Berserkers, Grue, Hulks and Seers. They can be killed in Helm's Hold during the mission "Sanatorium," and 'Quenching the Flames' and 'Foulspawn Foothold' missions in the Chasm.

Rift (after the first teleport you need to go down to the intersection, turn right and go to the next teleport - there will be several points where the necessary creatures constantly appear). Comrade Fijik added screenshots of the map below, for which we thank him.
