This is really easy if you know where to look!
There are only two pieces of 'lore' to find.
1. The first piece of lore is found when you first enter the new area in Chapter 1: In Defense of Gauntlgrym, look to your right and it will be on the right side on one of the tables in the corner. Don't worry if you miss it here on the way in, you can get it on the way out before exiting the area!
2. The second piece of lore is at the half-way point of Chapter 1--when you fight a boss in glowing red armor. Look behind him after the battle near the edge and you'll find the lore on the ground there.
If you accidentally complete the mission, don't worry! You can go back and do it again after completing the 3rd mission.

Are these achievements missable if you don't get it on a character? Can you revisit this area on the same character after completing it?

Yes, you can repeat it! I'll add that. You just have to talk to Bruenor again, just like doing the Dwarven Kings quest line.

Chapter 1: There are two lore entries to be found; one is at the beginning and one is at the end of the 'Arm of the Delzoun' zone. They are as follows:
Lore name followed by coordinates:
The Battlerager: 2293, 318 (to the right as you enter on the table with supplies)
Lans Thuliaga - Warrior: 3438, 435 (red banner behind the fire)
