You will also need to kill the 2 Bulettes in Reclamation Rock - Cultist Assault for this achievement:
The Halfling-Free Diet achievement in Neverwinter worth 35 pointsSave a group of halflings from being a bulette's lunch.

Just a note. This didn't unlock for me until after I completed the Halfling-Free Diet (kill the 2 bullette's).

Chapter 8: After the third group of enemies you face, take a right down the hill and you'll come across a group of earth elementals. Dispatch them for the lore entry; refer to the diagram below. The map does not rotate, so it looks like it's left, but in game it's to the right.
Lore name followed by coordinates:
