This is at the end of the Maze Engine Campaign's Chapter 12 Baphomet fight.
After defeating Baphomet, you are simply tasked with going to the exit a short distance in the Maze, and leaving.
You can leave right away, Don't!!
Wait here for all the limping adventurers to exit first.
THEN! DO NOT EXIT!
Go back to the starting campfire, and past it to where Makos is on the other side of a gate doing something..
Talk to him, he will come back through to the exit portal with you at which point you will get this achievement!

Chapter 12: After the demise of Baphomet and the Maze engine begins to fall apart; you'll find yourself in a hallway with your wounded allies. You must lead Drizzt, Linu, Regis, Knox, Celeste, Xuna and Minsc to the exit. Thankfully it's not very far; just take a right at the bonfire, then a left at the end of the hall and lead the team to the exit. Do not exit yourself!
Wait until the party is through the vortex and then head back to where you started. Here you'll find Makos, notify him that everyone is out and he too will join you to escape. Now both of you can run back to the vortex exit; let Makos reach it first. Once Makos says "Everyone's safe, now get out of here!" you'll net the achievement. Now you can make your escape!
