How to unlock the Third Time's the Charm achievement in Neverwinter - Definitive Guide
During the quest Temple Intrusion there will be a very dark square room with statue like enemies all pointing in different directions, seems its a different layout each time.
They are pointing at each other, they make a path which leads to each exit. It will be a random exit each time.
Edit: Suggestion in the comments is that the centre skeleton is the start for the path to the correct exit, see the second image below, it shows the centre skeleton pointing to the next skeleton on its right and so on.
As Kahless Clone suggests in the other guide, to open the two incorrect exits first you need to pick the exits with more enemies near them (enemies will awaken and fight you). I've tried to see if there are any other differences but there are the same number pointing to each door, I haven't noticed a pattern yet other than 5 pointing to one door are not as close as the others.
Open the last door which the remaining enemies are pointing at for the achievement. I earned this on my second try.

Aaaaand, second try. Left 5, straight 4, right 3, backwards 3.

At first I thought the same but I had five in each group on the try in the screenshots so yes I think that is random!
