How to unlock the Well They Shouldn't Have Died The First Time achievement in Neverwinter - Definitive Guide

There are about 5 places these dwarves spawn. The first 4 spawns are random. They can show up in different orders or not at all. #1 happened most runs but I have seen runs where it did not spawn.

Here is what they look like. This image is of spot #2:

1)In one of the puzzle rooms leaving the door. You may need to be fast to kill them before they leave. I don't know 100% if they disappear yet or not, but it's easy enough to get them when you see them.
NOTE: These will respawn if you go back into the room.

2) On the big set of stairs in the big room. They will be on the right against the wall on the 2nd set of stairs.

3) In one of the connecting hallways. They will be on the right side of the room at the back. There is just 1 and he is in a collapsed doorway.

4) At the bottom of the broken bridge. When you drop off the broken bridge, he will be against the wall behind you.


5) In the room before the final boss. Also 2 rooms after the 2nd boss. You will need to jump up a few stone blocks to walk into the next room. There seems to be at least 20 in this area.
*NOTE*: These dwarves respawn if you die at the boss. You will need to kill them if you die.


After you defeat the boss (and lag that comes with him), the achievement will pop.

I've found them in many runs, but usually I was able to find just 3-4 points. In particular, I've never seen them in the place 2. Can you please put a screenshot of the places you are referring to? Thanks.

I've found all of them and I can confirm that if you fail killing the boss at first try, you won't get the achievement unless you go back and kill all the dwarves again

UPDATE:
In the last 6 runs, I noticed that:
- the dwarves leaving the room always spawned in the first trap room (before Orcus)
- the dwarf working on the stairs is always located in the second flight of stairs (i.e. after Orcus)

Finally I got the achievement. It's ok if you kill all the dwarves and then your party fails the final boss fight, forcing to restart. The only important thing is that your party must not encouter the glitch that makes you spawn at the beginning of the dungeon. Then, when you kill the boss, the achievement will pop

I had the spawn glitch and it still popped, but we kill the dwarves each time.

Yeah, I mean that you won't get the achievement if, in order, you:
1) kill the dwarves
2) fail the boss fight
3) find the glitch
4) kill again the dwarves
5) fail again the boss fight (without glitch this time)
6) don't kill again the dwarves
7) succeed killing the final boss

Instead, if you do only the step 1-2-6-7 (i.e. you don't find the glitch and you don't kill again the dwarves) you will have your achievement unlocked

I just got the achievement despite us dying on the final boss first time, we went straight back, killed him, and achievement popped!

Finally got this with a random group. I didn't even kill any of them, someone else did :)

There are generally five spots were dwarves can spawn in ToNG: four are random, and the fifth is always at the end just before the boss...that hallway with around twenty dwarves.

Please Note: Should you die during the final boss, the dwarves in the hallway will respawn and you must kill them again.

The other random spots can be: off a puzzle room, the dwarves might leave quickly, so aggro them quickly! In the largest room of the dungeon, off to the right up a few sets of stairs. There can be one dwarf in a connecting hallway in an alcove. Once you drop off the broken bridge, in the area below should be a few dwarves.
