There are 6 total battles, 2 of which are story related and the other 4 are missable. If you miss any of these then you can always go back through Chapter Select and it will still count towards the achievement.
Episode 2

[STORY RELATED]
During the either stealthy or violent sewer segment you will encounter the “Badass Superfan” for the first time and will challenge you to your first Vaultlander battle
Episode 3

[MISSABLE]
When controlling Anu at Frans Frogurts, while looking for parts for the healing device at the start of the episode, check the fridge in the backroom to find the Superfan. He will challenge you to another battle.

Inside the Morgue as Anu, once you are left alone with the decapitaded corpse, check the cold locker in the corner to the right of the decapitated head to find the Superfan inside the locker.
Episode 4

The Superfan can be found inside the car trunk to the top left of the entrance in Fran's "New Shop".

You’ll encounter another Vaultlander battle to the end of Episode 4 when you encounter L0U13 again plugged into the power outlet as Fran.

[MISSABLE]
Once you’re reunited with Fran and L0U13 as Octavio you’ll reach Susan Coldwell’s office. Examine the Vaultlander on the red X on the floor to be challenged by the Superfan one last time.
Once you’ve won all these battles the achievement should pop not long after your final battle, no matter what order you won them in, just as long as you’ve won them.
