For this achievement, you simply have to let your character die.
The quickest and soonest way to die in the game is in Ep. 1 - Ch. 2 as Octavio after you agree to help L0U13 with his next job.
When you go to the apartment complex to help L0U13 with an assassination contract, tell the target you brought him pizza. Once you get him to open the door and you are given the next dialog options, simply press
or nothing at all to admit you're there to kill him and he will kill you instead.
There are many many other instances where you can die intentionally or otherwise, this is just the first one available.
