This can be done at the beginning of the game.


When searching for the eridium youll see a large safe that will have you put in a combination to unlock.


Purposely put the wrong combination in will unlock this achievement

Self explanatory. Intentionally miss any QTE action and this will pop almost immediately.

I did it when fighting The Devourer. Failed one of Octavio’s QTEs and it popped. (The one after it slimes you,) not sure if failing others in this fight will result in a death. Mine did not.
