To change the appearance for the ice mage you need to select the ice mage when starting a new game then you need to be in battle first and when it's the ice mage turn to battle just simply click on "change personality" and you will unlock this achievement.

The Ice Mage has an ability called Change Personality which will unlock this achievement automatically when used. If they don't start with the "Throwing Icicles" power that you need for @Freeze this is a great way to switch their abilities and get that.
