One of the easiest ways to get this is to start up a new season as the coach of the New England Patriots. Once the game starts, hit LB to go to the sim menu and simulate the game all the way to the next season. The team starts off with a 96 approval rating overall so unless the simulation makes some huge mistakes or takes plenty of losses, you should be fine by season's end.

If this does not unlock by then, keep simming. The off-season and draft count during this time so if you make it to the end of that time period with a 95 or over rating the achievement will still unlock.

You'll get this achievement on your way to getting the Best of All Time achievement. You can use any coach. If you have already gotten Best of All time but for some reason have note gotten this achievement, just sim one season using Tony Dungy and the Colts. Your overall approval rating should be 99 at the end of the season.
