How to unlock the Dear Diary achievement in NieR Replicant ver.1.22474487139... - Definitive Guide
The Recycled World isn't really a quest, its your mother's diary that unlocks after getting Ending A.
On the 2nd playthrough, return to your home and find the diary near the entrance.
The Recycled World consists of 3 doors, one after the other. Each door has some story text, followed by waves of shades. The first two doors are easy, with the third recommending the player be at least level 33. After each door, its possible to return to the real world and save, level, upgrade gear, etc, though its not really necessary.
I completed door 3 at level 31, with a level 2 Ancient Overlord, and didn't have any trouble until the final part of the section.
The last phase, you have to fight shades as you jump down broken bridge pieces, and at the bottom level, you fight "bosses", really just the big, armored shades with health bars. They come one at a time, and there are 4 total.
As long as you didn't get hit too much getting there, you should have enough healing to survive the last cannon shade. It'll have smaller ones shooting magic at you from the sides, but they're not a big deal.
I will say, the last bosses are fought on a small bridge, so be careful not to fall off too much. Their attacks hurt, and falling hurts too, so you could end up dying from being knocked off.

It does not have to be done on a second playthrough. It's available even after ending E. It also does not get harder from its appearance unlike enemies from Ending A ---> Ending B. If that's confusing, the simpler answer is: Wait as long as you want, you will never lose access and it will never get harder.
I did beat it on hard for the record and I had to be level 74 to take one hit from the final boss of the gauntlet. That was with 30%+ words applied to defensive stats.

I had problems with the heavily armoured shades in the second door on hard mode just after you get the weapon reward - if anyone also has issues, I eventually learned to stay back and range with magic (I used dark lance) until they hit the floor and then ground attack them with B. You need to ground attack them 3x to kill them.
Edit: I do not recommend you do the third door on hard mode. I'm level 49 and just got killed in 1 hit on full health after 40 minutes in the second to last section. By admitting defeat and lowering to normal difficulty I did it in 20 minutes and the damage scaling was ridiculous, normal was ridiculously easy.

After you finish the game for the first time you can start the second playthrough. When you're in your player's house there's there's usually 2 glowing orbs that you can pick up, but now there's gonna be 3 and the most left one if the 15 nightmares challenges, you need to complete them.
This quest becomes available on the Ending B path and will be available going forward from there. It consists of essentially a horde mode, killing tons of enemies for experience and money rewards. You can access this mission from the shelf next to the door in Nier's house, and must complete all three areas inside to unlock this achievement.
The Recycled World quests will open up once you've completed the game the first time. You'll actually automatically start these and will need to manually exit through the starting door to get out of them. There are 3 in total to get through, all of which are gauntlets to complete (for lack of a better term). The first will be open from as soon as the quests become available. When you complete that one, the second one will open. When you complete the second one, the third one will open. They each take anywhere from 15-20 minutes to complete, so this will take you an hour to do, give or take.
If you don't want to complete them when you're forced into them as soon as they open, you can re-access them via your Mom's Diary in your house in the Village. As soon as you enter the house, the first shelf you come to will have a marker over it:

As soon as you've completed the third and final quest/gauntlet, the trophy will unlock.
