How to unlock the e8a898e686b6e382b5e383bce38390e383bc achievement in NieR Replicant ver.1.22474487139... - Definitive Guide
You get this achievement by completing ending E. You can not miss this achievement as it continues directly on from Ending D. As this is all spoiler heavy for Ending D, I'll explain a bit more under the spoiler section.
Also, during ending E is your only chance to grab the disturbing "Daredevil" achievement.

Thank you for a slow introduction into how to do this, I followed your guide to a point and then closed it so I knew vaguely what to do but kept it a surprise for myself. Perfect.

This achievement is unlocked for Ending E, which is obtained automatically by playing through the story a 5th time after unlocking Ending E. It will seem like you're starting a brand new playthrough from the beginning, but it will quickly diverge from your original Ending A playthrough. Continue through the story to the end and choose "Yes" to complete it and restore your Ending D save file.
You will need to get Daredevil while going for Ending E. If you do not get it by the time you finish this small playthrough, you'll need to trigger Ending D again and go through the process of wiping your save files before you can try again.
After completing Ending D, you'll need to start a new game with a new name (your previous player's name will be locked). You don't need a full game playthrough for this Ending. Make your way through the first half and after you take down the mini-boss in the Aerie, Kainé will do her dramatic "just let me die here" schpeel. This time, when you run up to her to get her back on her feet, you'll start a new gameplay sequence and take over as Kainé. Progress through her short story and when you reach the end, you'll be asked if you want to recover the person you cherish. Choose "yes" to get Ending E.
After the non-skippable credits roll, you'll be asked to remember the name of "the person you cherish". Type in the name you originally chose for your character prior to your save files being wiped. This will restore your save file and when you hit continue at the main menu, you'll start back up at the start of Part Two, on the path to complete Ending D. If you had any incomplete side quests in your Grimoire at the time of your save file being wiped, you'll need to pick them back up again before you can complete them.
You'll also be given one additional story-related quest to pick up a fancy new weapon that is incredibly OP. Follow the red "X" on your mini-map and you'll find the weapon waiting for you.
