This will unlock naturally when you complete your 2nd playthrough of the back-half of the game. Once you save your game and are asked if you want to leave that screen be sure select "OK." If you instead close the game and load your file from the main menu to continue on to Route C this achievement will not unlock.

This unlocks automatically upon completing the game a second time on the same save file where you earned Ending A.

Story-related and cannot be missed.
Ending B will happen when you complete the game a second time after choosing to "continue" from your Ending A save file. You'll only need to do the second half of the game again, so if you want to skip everything you've already seen when doing Ending A, this step will take anywhere between 1-3 hours. When the non-skippable credits finish, be sure to save your game when prompted. When you're at the main menu, hit "continue" and reload your save file. You'll start back at the beginning of Part Two and can work on getting Ending C (Thank You
