In short, this is the boss fight after the Shadowlord right before ending C or D. You must already have all the weapons to get to this boss fight. It is very unlikely to get this achievement without skipping the cutscenes, so I highly recommend creating a separate save file at the top of the Lost Shrine just before you enter the Shadowlord's castle.

this fight will happen if you are trying to get all of the endings and after the 33 weapons. you will be overpowered enough to kill Kaine with that time limit. however, the game doesn't stop the time counter from ticking while you are in a cutscene. The best option is to skip all of it and watch them later on youtube for the sake of getting this achievement.

For all of the boss rush achievements, I would highly suggest turning the difficulty down to Easy and turning on the Auto Combat. You can edit your magic to add a word and increase its damage as much as possible, same with your melee attack power. Doing all that, you should be able to get all these achievements on your first attempt with little trouble. If you don't get any during your Ending A path, you'll have multiple additional attempts when going for the other endings, and your level will only increase with each attempt. Kainé will go through a variety of attacks, mostly alternating between trying to punch you in the face and drown you in orbs. Evade her attacks as best possible and attack her at any and every opening you have. It'll take a bit to get her down, but as long as you're staying on top of it, you should be able to get this within the time required. If you don't get this on Ending C, it'll be much easier to do on Ending D.
