In order to unlock this achievement, you must hang out with Germ (the weird bird guy) whenever possible.
You meet Germ on Day 2, and, from that Day onwards, you should talk with him. Starting Day 4, you'll find him near the Food Donkey (all the way left from town).
The main requirement for this achievement is to play all 3 "hang out scenes" with Germ. All three start by talking to him near the Food Donkey on the following days:
- Day 6, on the Food Donkey Parking Lot.
- Day 9, under the bridge at town entrance. This is needed for another achievement.
The Secret Achievement achievement in Night In The Woods worth 41 pointsContinue playing to unlock this secret achievement
- Day 11, Germ's house.
The achievement should unlock as soon as the third scene finishes.

I talked to him on day 11. He did not hang out. He just left. I could not find his house. What gives?

Seems like you need to make sure you go find him at every opportunity for him to take you to his house on Day 11

Hang out with Germ at every opportunity from Chapter 2 onwards. He can be found outside of the derelict Food Donkey store most of the time.
Achievement should pop when you speak to him during the story segment in the final chapter.
