On Day 7 (Harfest), go to the cliff that is right from the church, and talk to the three goth kids in there.
When they make you a question, answer "A shipwreck". The answer to the second question isn't important.
You'll need to find three pentagrams for this achievement.
You can find the first from Day 9 onwards. Enter the building where Gregg lives (two right from Snack Falcon) and take the stairs from the roof. Go left until you find a door in the roof of another building. Inside, go all the way to the left to find the Pentagram.
The second one appears on Day 10 on the final friendship quest. There's two possibilities:
If you bonded with Gregg, you'll go to a donut place. When ordering, choose "Doomnuts", and one of them will have the pentagram. Pick it up.
If you bonded with Bea, you'll go to a party. You'll have to talk to a girl named Bombshell, and get her to teach you her secret handshake.
The third Pentagram appears on Day 12. I won't spoil anything, but it is unmissable as long as you talked with the goth teens on day 7.
The achievement will unlock at the beginning of the Epilogue.

For me the second pentagram seems to have appeared as soon as I chose the "Doomnuts" donuts because a journal entry has been made about it right after.

I don't know why the solution goes cryptic on the final pentagram because anyone reading this shouldn't be worried about spoilers. The third is made of wood beams in the ceremony room in the mine. You don't have to interact with it, just walk past it to go to the next room.

Talk to three teens on the cliff edge near the church on Halloween, and they will give you a quest to find three pentagrams (answer "shipwreck" to their riddle).
The pentagrams can be found:
- On a poster in the secret stage (top of tall building near Snack Falcon, accessible from the rooftop of Gregg/Angus' apartment building).
- Order Doomnuts with Gregg OR speak to the girl at the back of the party with Bea.
- Automatically found in the mines near the end of the game.
