You need a minimum of 8 playthroughs and tag missions to finish the game 100% and this achievement might come naturally. If not or you want to simply speed it up, do the following After at least finishing the game once:
-From the main menu enter tag missions
-Select any mission on the Acolyte difficulty
-Select Rachel as a character and any partner(doesn't matter)
-When the misison starts go to any of the corners, Aim with
and spam 
to finish enemies and you will get it in no time.
-When the mission finishes repeat the process again.
The achievement is cumulative between campain/chapter select/tag missions and the achievement tracker works really fine.
