Simply receive any type of medallion in any of the survival missions between 17 and 24. "First medallion" is somewhat misleading as any karma attack mission medallions will not count for the achievement. The secret description won't become apparent until the achievement has been unlocked.
This achievement has nothing to do with your kills accumulated as the specific type of medallion that you receive is based on your Karma score after finishing a survival mission. 2,000,000 points appear to be necessary for a gold medallion.
