A counter attack is performed by blocking and then attacking as your enemy attacks. This is done by holding LT and then pressing X or Y at the right moment.
During Chapter 2, just past the river, you'll pick up a scroll explaining this technique. While it may be possible to unlock this achievement before this point, performing one counter attack once in possession of the scroll should be sufficient.

First, get the Counter Attack scroll, which is just passed the river in Chapter 2. Then, hold 
to block and press 
or 
once you hear the clang of your sword.
You can't get the achievement until you get the scroll I tried doing that but it didn't work. I got the achievement on my first try after finding the scroll, however.
