How to unlock the Refashion achievement in Nioh - Definitive Guide

To refashion an item go to Blacksmith -> Refashion.

You can give gear in your inventory the appearance of other gear that you have previously found. You can even give light armor the look of heavy armor without changing any stats or equip weight.

This is done between levels at the starting point (shown on the map as a large house). There, at the blacksmith you can change the appearance of any weapon. First, select the weapon to change, then the second weapon, the type of which you want to copy.
All this costs gold. Gold drops from enemies, drops from broken items, and is earned by selling items to the blacksmith.
