This is the bad ending achievement. I got this without having to replay from the whole game. If you follow the guide to get the good ending you can do the delete console save trick (ie go straight to dashboard without closing the game and delete your save using the manage game option). Reloading the game after doing this will put you back at the scene where you are crawling along the bridge. Shoot Salma and you will get the bad ending. Absolutely no other choice matters.
